چکیده

UNLABELLED A 57-year-old male with type 2 diabetes mellitus presented to the emergency department with sudden onset of chest pain. Shock status and considerably low right arm blood pressure were detected. The patient underwent contrast-enhanced computed tomography (CT) which revealed dependent contrast pooling and layering of contrast material within the inferior vena cava. Post-processing CT angiography depicted total occlusion of the left main coronary artery. Sudden cardiac arrest developed after CT examination, and following emergency coronary angiography we confirmed the diagnosis of left main coronary artery occlusion and cardiogenic shock. Clinical physicians should recognize these CT findings of imminent cardiovascular decompensation and provide prompt medical management to prevent further patient deterioration. KEY WORDS Cardiogenic shock; Dependent pooling; Layering; Left main coronary artery disease.

Total revascularization for an epsilon right coronary artery and severe left main disease combined with profound cardiogenic shock

RATIONALE Severe left main disease combined with right coronary artery occlusion was rarely encountered in our daily practice. Percutaneous coronary intervention in these patients was most challenging due to high probability of hemodynamic changes. PATIENT CONCERNS Here, we report a 67-year-old man with Non-ST-Segment Elevation Myocardial Infarction (NSTEMI) and profound cardiogenic shock and...

Left main coronary artery thrombosis in a 19 year-old patient.

Left main coronary artery occlusion is a very rare entity that often results in death. It usually manifests as acute myocardial infarction (MI) with cardiogenic shock and fatal arrhythmias. Here, we report the case of a 19 year-old patient who presented with acute anterior MI secondary to left main coronary artery thrombosis. There were no classical risk factors for coronary heart disease in th...

Electrocardiographical case. Beware of this ECG in patients presenting with cardiogenic shock.

A 64-year-old Chinese man presented to the Accident and Emergency Department with chest pain and was found to be in cardiogenic shock. The electrocardiogram (ECG) showed features of acute myocardial infarction due to left main coronary artery occlusion, which had no ST elevation in precordial leads. Emergency coronary angiogram revealed left main coronary artery complete occlusion. Percutaneous ...

Acute myocardial infarction in an 80 year-old woman caused by left main occlusion with concomitant chronic total occlusions of right and left coronary artery: successful treatment with percutaneous revascularization.

Although acute left main coronary artery occlusion is rare, it carries a very high mortality rate. Following the encouraging results of trials testing the effect of primary percutaneous coronary intervention, more cases of left main stenosis are treated as culprit lesion in acute myocardial infarction. Few cases of primary percutaneous intervention on left main occlusion have been published. We...

A case of chronic left main trunk occlusion treated with off-pump coronary artery bypass grafting

Ischemic heart disease presents with various symptoms, and chest pain is a chief complaint. Occlusion of the left main trunk often results in serious symptoms such as cardiogenic shock. However, while extremely rare, left main trunk occlusion can present with only slight chronic symptoms. In the present case, a 62-year-old woman with few risk factors for ischemic heart disease visited our hospi...
